Our goal is open communications. Clearly stated and understood policies are essential for you and your child to feel comfortable. We will inform you about your child’s dental health and needs and explain the procedures and purpose. The costs will be explained. Please read the following to help you understand.

APPOINTMENTS

Are by appointment. Emergencies are taken but please call first. For after hour emergencies call the office phone number for instructions.

  1. A responsible adult must accompany your child for each visit. A parent or legal guardian must complete a health history form and give consent to treatment.

  2. At the initial visit we will perform an examination and provide a treatment plan with associated costs.

  3. Children under the age of six have better behavior in the morning. Therefore we schedule those patients in the AM. Sedation requires fasting so morning appointments are preferable. Since most of our patients are school age it may be necessary to schedule some appointments during school hours.

  4. As a courtesy, we will confirm your appointment by phone the day before. In the event you cannot make an appointment, please give us adequate notice. Not showing or late cancellations are a problem in which case you may be required to place a deposit for a future appointment.

FINANCES

  1. Payment for services is due at he time of treatment. We accept Visa, MasterCard, Discover, American Express, checks or cash.

  2. If you have dental insurance, we will accept it if you assign benefits payable to our office. You must provide us with a current dental insurance card. Your child’s eligibility for benefits must be verified. If the fees for dental services exceed a certain amount, a predetermination may be required by your carrier

  3. We will estimate the insurance coverage and you must pay the difference. It is only an estimate and not a guarantee of payment. Even a predetermination of benefits from your insurance is not a guarantee of payment. You are responsible for your bill not the insurance company. You are responsible for charges not covered by you insurance. You will be reimbursed for any overpayment.

  4. Insurance is complicated. Insurance plans vary by employer and carrier. Co-pay, deductibles and fee adjustments are common practices for insurance companies. You may not perceive it as fair or what you thought your insurance was. Remember, we have no control over that. That is between you and your employer. It is not our policy or responsibility to pursue payment from your carrier. Some insurance companies are not cooperative. We deal with insurance on a regular basis. We will do all we can for you.

  5. If you do not have traditional insurance, your coverage can vary widely. If you are referred by a family dentist, you may be required by your carrier to have a referral form from that dentist.

  6. We will not carry balances over 90 days. If insurance has not covered your portion, you are responsible for this balance immediately or it will be turned over to collections.

  7. If you assign benefits to us and the insurance company sends a check to you by error, you are responsible for paying us by either sending us that check or some other form of payment immediately you.

  8. Communication with us can resolve insurance problems. If you receive communication from your insurance company, contact us. Ignoring issues makes it more difficult to resolve later. Remember we are there to help.
